The key with the second diaper would seem to be making sure that your hand is on it all the time lest the removal of pressure allow the diaper to slide off of a wiggling baby. Or even just a lessening of pressure allowing for a corner to slip in the seconds before disaster strikes.
Yes, experience here is hard earned.
